We have a great client in Syracuse, NY who is looking to expand their Home Care RN team. They are looking for a RN to work in the Syracuse area Monday-Friday (1st shift) with their BS in Nursing coupled with at least one year of experience. This position has very little overtime and less then 24 hours a year of weekends! This is a great opportunity if you like to work independently, are self-motivated and enjoy having control of your schedule!

Requirements:
- Current NYS RN license
- Valid NYS driver's license and a vehicle to travel Onondaga County
- Minimum one year nursing experience in acute care, LTC, or Home Care.
- Provides quality care through assessment, development, implementation and evaluation of individualized participant care plans.
- Teaches and counsels participants and caregivers as needed regarding maintenance of health and prevention of illness or complications.
- Delegates tasks appropriately and supervises, instructs and guides nursing and paraprofessional staff in providing high quality nursing services.
- Contributes as a member of the PACE Interdisciplinary Team. Acts as a liaison between the participant, the family/care givers, the home care supervisor and the PACE Interdisciplinary Team.
- Provides leadership among co-workers
